COMPASS NEURO

NAVIGATION AGREEMENT

This Navigation Agreement sets forth the terms and conditions of your receipt of navigation services from Compass Neuro, LLC (“Compass Neuro Navigation”). You therefore agree as follows:

Compass Neuro Navigation is an expert-led advisory relationship designed to help individuals and families move through neurological uncertainty with clarity and confidence through four (4) different pathways (each a “Pathway”) as set forth below.

Neither Compass Neuro Navigation nor its owner, Dr. Andrea Douglas, provide clinical care or medical services through Compass Neuro Navigation. You will not have a physician-patient or treatment relationship with Dr. Douglas by virtue of receiving services from Compass Neuro Navigation. Compass Neuro Navigation is strictly designed to help individuals and families navigate the challenges of neurological uncertainty. If you desire clinical neurological services, you need to develop a physician-patient relationship with Dr. Douglas through Compass NeuroSpine, PLLC or with another provider. As Compass Neuro Navigation does not provide clinical services, it does not bill Medicare or any insurance company. You agree to pay for Compass Neuro Navigation’s services in the amount applicable to your selected Pathway. The pricing for each Pathway is all-inclusive for the services included in the applicable Pathway.

Navigation Pathways

Pathway Option 1: Introductory Pathway

A focused introductory consultation for individuals or families seeking early clarity before choosing a fuller navigation Pathway.

Includes:

• 30-minute expert strategy session

• Discussion of diagnosis, recommendations, concerns and goals

• High-level guidance on next steps

• Recommendation regarding the most appropriate Pathway, if further support is needed

Please note, this Introductory Pathway does not include detailed records review, written synthesis, or ongoing access.

Fee: $500 | single engagement

Pathway Option 2: Focused Neuro Guidance

A single, focused engagement for early clarity.

Includes:

• Review of relevant medical records or imaging

• Expert interpretation of a diagnosis or recommendation

• Clear written synthesis outlining options discussed and next steps in your care journey

Best Suited For:

• A specific question or decision point

• Early uncertainty

• Determining whether further support is needed

This pathway is an individual-based, single engagement, does not include interaction with any of your family members, and does not include ongoing access.

Fee: $950 | single engagement

Pathway Option 3: Comprehensive Neuro Navigation

Structured guidance for families navigating ongoing complexity.

Includes:

• One Primary Client

• Support for up to two additional adult family members

• Medical record collation and synthesis

• Diagnostic and treatment pathway strategy

• Preparation for key clinical visits

• Post-visit debriefs

• Structured asynchronous access over 12 weeks

Best suited for:

• Families coordinating care across providers

• Situations unfolding over time

• Those seeking clarity without rushing into treatment

Fee: $5,000 | 12 weeks

Pathway Option 4: Priority Household Neuro Partnership

Our most comprehensive navigation relationship.

Includes everything in Comprehensive Navigation, plus:

• Priority response times

• Real-time support around key appointments

• Enhanced availability during critical decision windows

• Ongoing strategic oversight across diagnoses and providers

Best suited for:

• Time-sensitive or high-stakes decisions

• Surgical consideration

• Multi-provider or multi-diagnosis complexity

Fee: $7,500 | 12 weeks

The applicable fee is due upon signing this Agreement.

Note:

1. If you desire to transition from the Introductory Pathway to Compass Neuro Navigation - Focused Neuro Guidance, $250 from your Introductory Pathway fee shall be applied to your Focused Neuro Guidance fee. In order for the credit to apply, you must transition within 7 days of enrolling in the Introductory Pathway.

2. If you desire to transition from the Introductory Pathway to the Comprehensive Neuro or Priority Household Neuro Pathway, the full $500 Introductory Pathway fee shall be applied to your higher level Compass Neuro Navigation Pathway. In order for the credit to apply, you must transition within 7 days of enrolling in the Introductory Pathway.

3. If you desire to transition from the Focused Neuro Guidance option to one of the 12 week Compass Neuro Navigation plans (Comprehensive Navigation or Priority Household Neuro Partnership) with the same individual as primary client, the full $950 Focused Neuro Guidance fee shall be applied to your higher level Compass Neuro Navigation Pathway. In order for the credit to apply, you must transition within 14 days of enrolling in the Focused Neuro Guidance option.

Term: This Navigation Agreement shall begin on the date set forth above and shall expire upon the conclusion of the single engagement described in Pathway 1 or 2 above or the conclusion of the 12-week period described in Pathways 3 and 4 above, as applicable.

If you choose Pathway 3 or 4, this Navigation Agreement may be terminated by you or Compass Neuro with fourteen (14) days advance written notice. If you or Compass Neuro terminate this Navigation Agreement for any reason, you will be entitled to a prorated refund of the fee that you paid.

Entire Agreement: You agree to the terms of this Navigation Agreement, all of which are expressed herein. There are no promises or representations between you and Compass Neuro except as set forth herein.

Notices: Any communication required or permitted to be sent under this Navigation Agreement shall be in writing and sent via certified U.S. mail or recognized overnight courier to the addresses set forth below. Any change in address shall be communicated in accordance with the provisions of this section.

Billing: Fees are due upon signing this Navigation Agreement. Failure to pay all or any portion of the Fee may result in termination of this Navigation Agreement following written notice from Compass Neuro.

Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the State of Connecticut without regard to Connecticut’s choice of law provisions.
